I understand I will be required to complete a "Parental/ Guardian Consent Form and Liability Waiver" with an included "Photo Release Statement" prior to my child attending SEARCH. I understand that the required forms will be emailed to me prior to my child's SEARCH retreat. 

Furthermore, I understand  in order to be eligible for a refund,  all reschedules/ cancellations must be done at least 10 days in advance of my child attending their scheduled SEARCH retreat. After these 10 days, refunds will not be available. I understand that this is due to the financial obligation the Diocese of Nashville has with the retreat center as final numbers are turned in at this point. 

Lastly, I understand that SEARCHERS should not bring a cell phone to the retreat. If they are brought,  the adults will hold on to them for the entirety of the weekend. I also understand that drugs, alcohol, and tobacco products of any kind (including vapes) are not to be brought to SEARCH. If a participant is found with these items, the parents will be called and they will be sent home.
